How would Common Courtesy interact with an infinite loop such as Aluren + Horned Kavu? Would you have to ask permission every loop, or just work asking permission into the loop?

I would assume it's the smae as all other loops. You declare how many times you want to do it and it is done. if they want to do somthing about it let them say so.

Although I'm not entirely sure what you mean, I think I know, and that's good enough for me.
Odds are if it's an infinite loop there's little they can do about, and anything that would be done would be done at the beginning or as a result of the loop, for instance, creating infinite mana to Enrage a creature, then shocking the creature or countering the spell or something.
It makes sense to just let the loop run as long as the person likes, and make them ask permission as soon as they run something out of the loop.

Jacen is correct. when confronted with a loop, you go through the process once to make sure what all will happen, then the person controlling the loop states how many times he/she will repeat it. you, if you have a way to stop it, can name how many times you will allow it to happen, whether that be 100 times or 0 times.
